Alphabetical indexes currently search on header and footer, which IMHO should
be avoided (or, at most, be optional), as these regions often include chapter's
titles (or document title), author's information... These concepts are very
likely to be part of an alphabetical index due to their importance (if it's an
important concept it will be likely included in the document's title) but if
they are repeated in every page because they appear on the header or footer,
alphabetical index becomes useless.
