Steps:
1) Open attachment 127291
2) Zoom to a level where you can see 3 pages per row
3) Notice that the Table of Contents goes along pages 1 and 2
4) View > Web
5) Wait until repagination finishes and the page number in the status bar says
'1 / 1'
6) View > Print or Normal
7) Notice Table of Contents now appears at the top of page 3
Regression as it fine in 4.2.
