Please don't report multiple bugs in a single report.
And it would be nice to see a broken minimal MM result.
I've attached a broken one myself, but this seems to have general problems with
the embedded PNG image, but otherwise is correct. I'll open a new bug report
for the PNG problem.
For an explanation of the empty pages see the duplicate bug report.
If you still get the crash on Windows 7, open a new report.
Can you try to reproduce the crash using a daily build available at
http://dev-builds.libreoffice.org/daily/master/ ? More

For this kind of work, LO has a label generator (File -> New ->Lables). No need
to seperate the numbers into columns, as you can use multiple datasets per
page. Use synchronized labels, so you just prepare one card and LO copies the
content into the others.
