I could repro with the file.
I deleted all that was on page 80. Then I just inserted dummy text with dt +
F3. Then I went and updated the ToC many times. I went back to look at pages
after 80 and there were a lot of empty columns.
Everything is possible, so creating the file in AOO might cause the problem.
It would be great, if you found a way to reproduce the problem from scratch in
a new file created in LibO.
