Wouldn't it be more useful to make the user-defined indexes more flexible?
Right now, user-defined indexes are fixed to sort by page number. If they were
given options to sort by an index key and merge duplicate page numbers then
they would emulate alphabetical indexes and the users can create as many as
needed.
