Without a sample mysql DB (SQLdump or at least the DDL to create and add data),
and a corresponding ODB file containing the forms and macros, there isn't much
I can say at the moment.

Setting to NEEDINFO
@tommaso : please also state whether you are using the distro-provided versions
of both LibreOffice and the mysql connector extension, or whether you have
installed the TDF package for LibreOffice and a mysql connector extension from
somewhere else (if such a package even exists). The version number 1.0.4-3
doesn't ring a bell with me, so I'm uncertain where you have got that from.
