Could confirm the buggy behavior.
Open a form in a database - you don't need any table.
Create a checkbox.
Label will be written, checkbox doesn't exist any more.
Doesn't work in LO 5.3.0.2, works in LO 5.2.4.2
All tested here with OpenSUSE 42.1 64bit rpm Linux.
