Some sort of documentation addition should be made explaining how to create a
TOC with page numbering by chapter. Looking back, it would have been easier for
the writing community to grasp a different approach to generating the TOC.
Changing the approach at this point would introduce confusion or perhaps break
documents written prior to the change. A documentation change is preferable to
a code change that might cause failure in existing documents that depend upon
the current technique.
