I can confirm the overline problem just by opening the Epub file in Apple's own
iBook reader app (whether or not that is a suitable reference implementation is
another matter, about which I know nothing).
For some weird reason, the table is also split. I imagine that this relates to
the .span2 style rule that you mention.
