Basically, the Pivot Table dialog window areas turn black when you drag and
drop a cell header (or field - I used the bibliography database as a source),
which then masks the entry you have just dragged and dropped, but also all of
the potential entries in the list of draggable fields. You have to click
somewhere in the black area in order to get the menu entries to re-appear (one
at a time when there are several of them)
