Marking as NEEDINFO until a document is provided. I'm not sure how to get the dialog to appear to reproduce this and prioritize. We'll leave the bug in NEEDINFO status for 30 days at which point we'll mark as INVALID due to lack of activity. Whenever you can get the document attached changed to UNCONFIRMED again (either from NEEDINFO or from INVALID).
