Problem description: When inserting a cross reference to a heading: number,
number(no context), number(full context) are all inserting only the number. It
is not possible to enter a heading with full context.
Steps to reproduce:
1. Insert a cross reference to a numbered heading. Choose full context options.
2. See that only the number is inserted.
Current behavior:
No context entry in any options
Expected behavior:
Full context entry when this option is selected.

Browser: Mozilla/5.0 (X11; Linux i686; rv:15.0) Gecko/20100101 Firefox/15.0
