Thanks for looking into Eclipse's trademark policy. Unfortunately
LibreOffice/TDF is more restrictive and states 'Don't change the logo by e.g.
adding additional graphic elements to the "Document Symbol".' [1]. 

And for the where-to-place question you should give more details. If the icon
is used by an extension it goes into a folder and is referenced in the
configure.xcu with for example "<icon> <default
xlink:href="icons/Eclipse.png"/></icon>".
