It has nothing to do with the special callout shapes. But is can be reproduced
with simple non-primitive shapes.
The error occurs in the case you have a command sequence like "M L N M L" where
the set of subpaths is finished by the command N and then a sequence M L
follows. If you remove the N so that there is only one set "M L M L", the lines
are drawn correctly. I'll attach those examples.
