This bug is easiest to describe as a recipe to reproduce it. Try spell-checking
a document like this:

foo foo

where "foo" is a word not in your spelling dictionary. Both occurrences of
"foo" are underlined, as you would expect, before spell-checking starts.

The spell checker prompts with the first occurrence of "foo" highlighted, as
you would expect. In the dialog, click "Add". Now the second occurrence of
"foo" is presented, which is wrong: it has been added to the dictionary.

A clue to the bug is that if instead you spell-check a document that looks like
this:

foo
...lots of text...
foo

then it works as expected. In other words, the bug only occurs when the word
added to the dictionary appears more than once in the spell-checker dialog's
mini window on to the document.
