I am afraid this document is pathological. The pictures in it are not
vector-format, nor are they bitmaps - they are "vector-format" with millions of
tiny rectangles.
Somebody ran this document through a very bad converter.
About the only thing we could do would be to error out when detecting such
PDFs.
