The root of the problem seems to be that the heading paragraph styles are
inheriting from -none- instead of
<w:style w:type="paragraph" w:styleId="1">
<w:name w:val="Heading 1"/>
<w:basedOn w:val="Style14"/>
<w:style w:type="paragraph" w:styleId="Style14">
<w:name w:val="שורת כותרת"/>
<w:basedOn w:val="Normal"/>
