Description:
The setting of the "Create PDF form" checkbox is not saved during multiple
Steps to Reproduce:
1. File -> Export as PDF...
2. In the "General Options" tab, activate/check the "Create PDF form" checkbox.
3. Export document as PDF.
4. File -> Export as PDF...
5. Check the setting of the "Create PDF form" checkbox.
Actual Results:
After opening the "PDF Options" dialog a second time the "Create PDF form"
checkbox is unchecked again.
Expected Results:
Preserve the setting of the "Create PDF form" checkbox.
Reproducible: Always
User Profile Reset: Yes

Not sure if this related in any way or not but there are 3 GUI object in this
dialog having 'c' as keyboard shortcut.
