To see the problem:

1.  insert an index entry  (Insert > Table of Contents and Index > Table of
Contents, Index, or Bibliography - Index Entry

2.  edit this entry  (right-click with cursor in index entry, choose "Index
Entry...")

3.  Click on "Help" Button, 
4.  Place cursor in different fields, check boxes and press F1

Actual result (for both Help button and F1):
