This issue is a duplicate of:
Bug 131524 - Date format in a cell with conditional formatting turns to money
and no bug, cause conditional cell format handling changed with LO 6.3.
You need to adjust cell styles "ConditionalStyle_2" and "ConditionalStyle_3"
cell format from percentage to date.
