the "Tools/Extension manager" menu item cannot be assigned to any keyboard
shortcut or toolbar button.
if you select "Tools/Customize/Keyboard" or "Tools/Customize/Toolbars/Add" you
will find that Extension Manager never appears in any of the available command
lists.
