Summary: Crash when creating a form with Wizard

After create a fresh table, I tried to create a form with Wizard.
I choose some columns of the table(I tested with one columns, with all columns,
it always crash...). All other steps are just Next.
In the last step(Set Name) after I click on Finish, this is the log on the
terminal:
