The expectation is wrong. A shape or image anchored to header is not the
background of the page. Setting the wrap properties to "run-through" and
"background" describes only the relationship of shape or image to the text.
The stacking order in Writer is
- object in front of text
- text content in paragraphs or tables
- object behind text
- fill of paragraphs or tables
- fill of page

If you want an image to be behind all content, then you need to make it a page
background.
