Is this a clue? I used Word 2003 to round-trip the file as DOCX, and Heading1
doesn't have an ilvl value.

</w:numPr>
(In the DOCX, the numbering/level is direct assigned to the paragraph - perhaps
done by the round-tripping? Using a new version of Word to RT might be better.)
