It's not clear whether you have been able to open the original file yourself now, or just that my "fixed" version of the file was enough for you. Marking RESOLVED but if there is still a problem in LO can you update this bug? I have tried to create a new document (LO 3.6.2.2 on Ubuntu 12.10/64, LO 4.0.0.3 on Win7/64) and save it with the print preview dialog open but cannot replicate your results (Save icon and menu item are grayed out.)
