When writing an RTL document, and using a multi-column index or table of
contents, you obviously need that ToC or Index to have a RTL column layout,
i.e. content begins on the rightmost column and continues leftwards.

I was flabbergasted today when told (on IRC) that, apparently, there is simply
no support for that right now.

Note that this should also have been a mjaor failure in importing MS-Word
documents - but those don't seem to have a ToC imported that well to begin
with.

Please fix this ASAP - I need a two-column ToC :-(
