My field listing is filled with dozens, or hundreds, of entries for the items
in the ToC - as though they were cross-reference fields I had inserted. I don't
want that! In fact, I very much doubt any user wants to have entry-level
navigator visibility into the ToC, as opposed to the entire ToC being a single
thing you've entered.
So, I suggest generated intra-ToC/Index entries not show up as individual items
in the Navigator.
