1. Insert - Table of Contents and Index - Insert Entry
2. In "Insert Entry" dialog, choose Index: User-Defined
3. Give any string for "Entry"
4. Press "Insert"
5. Place cursor on the inserted entry.
6. Execute .uno:IndexMarkToIndex (Index Mark to Index)
(you probably have to add it to a menu or toolbar or key shortcut from Tools
- Customise)
