Open Insert - TOC/Index dialog, Type tab, with Type "Table of Contents"

In "Create From" section, proposal:

"Additional styles" -> "Selected styles"

Main reason: It is possible to make a TOC using only the "Selected styles"
option (so it is not "Additional").

Elaboration:
1. Presumably "Additional" was chosen in relation to "Headings" (with the
assumption that "Headings" was always selected, so one could "add" some
styles?). Also, if "Headings" is chosen, then it is not possible to modify
their index level using "Assign Styles".  (iow, if Heading is selected, then
the "Additional styles" really are "additional").

2. But if Headings is not checked, then "Additional styles" could be used to
make a TOC using Heading PS (e.g., "Heading 5" and "Heading 7" or just "Heading
4").

3. Similarly, an (ToC) Index could be created from non-Heading PS, without
using "Heading"

 => "Selected styles" is part of the "Create From" section, where this option
alone could be used to create a ToC (index). 


NB.  with type "User-defined index", this same control is labeled "Styles", but
there is no "Headings" in that dialog.  "Selected" signals that choices are
involved (in contrast to "Headings", which gives no choice).
