My issue finally resolved to this: when a caption was added to an image, a
frame was implicitly created, of which I was unaware. When I moved the image I
could only do so within the frame, leaving the caption behind. This is because
I approached the frame from the right. After filing the bug I discovered that
approaching from any other side allowed me to select the frame & so move both
image & caption together. 
Side note: I have selected 'Do not draw frame borders' (or similar) for a much
clearer presentation. These may have alerted me to the creation of the new
frame.
To recap, if I approach the mouse cursor to the Frame+Image+Caption from the
right, I can only select the image. From left, top & bottom the cursor
encounters the frame first, then the image. 
If that counts as a bug, then it's a bug. I hope I have been able to clarify -
I realise how hard it is to communicate these things.
