Description:
In the dialog box Tools | Footnotes and Endnotes | Footnotes, the labels
"counting" and "start at" are reversed.

Counting
Select the numbering option for the footnotes.

Per chapter
Per document

Start at
Enter the number for the first footnote in the document. This option is only
available if you selected "Per Document" in the Counting box.

But in the actual UI, "counting" is a number and "start at" is a dropdown
selection list with per page/per chapter/per document. Also, in the Endnotes
tab,  "Start at" requires a number, and there is no option for "Counting."

(This may be a separate bug/enhancement request, but it is very weird and
confusing to separate two options related by dependency -- start at and
counting, the latter of which is dependent on the former, by two unrelated
options -- before and after -- and to put the dependent option, start at,
*before* the option on which it is dependent, counting.)

Steps to Reproduce:
1. Go to Tools | Footnotes and Endnotes.
2. Click the Footnotes tab. 
3. See that "counting" requires a number and "start at" is a dropdown listbox.
4. Click on the Endnotes tab. See that "start at" is where "counting" is in the
Footnotes tab.

Actual Results:
The two labels are reversed.

Expected Results:
The labels should be properly assigned.
