To duplicate:

1. Create empty Writer doc.
2. Insert a text box. Copy and paste it a few times. The copied form elements
will all have the same name "text box 1" (or similar, depending on language of
installation).
3. Save doc as PDF form (do not click in "allow duplicate field names".
4. Open the form in a PDF reader and fill it out. Save form as a copy.
5. Open the saved copy in a PDF reader. It will be corrupted, and not display
the filled in stuff in a good way since the text boxes all have the same name.

Would be reasonable to make Writer check for duplicate field names and warn
about this before exporting? I'm not sure if that would be a good feature ...

If "allow duplicate fild names" is crossed then everything is of course ok.
