Attached simple example file contains a few pages and a TOC.
The accessibility sidebar shows warnings like "Hyperlink text is the same as
the link address “#__RefHeading___Toc844_85589850”." which is due to the
special makeup of the TOC, but there is nothing a user can do about it.

1. Open attached document
2. Check the Accessibility Check sidebar
-> should not have warnings about Hyperlink text is the same as the link
address.
