Of course, it may be specific to your document, if it's the case, we need: the document sanitized (see https://wiki.documentfoundation.org/QA/Bugzilla/Sanitizing_Files_Before_Submission) and above all, a step by step process to reproduce this but since you said at the initial comment you don't know what triggered this, it's gonna be hard.
