Here is what the Chicago Manual of Style 16th Edition says in §1.48: "Footnote reference numbers or symbols should not appear anywhere in the chapter display. A note that refers to the chapter as a whole should be unnumbered and should precede the numbered notes, whether it appears on the first page of the chapter or in the endnotes (see 14.47). A chapter epigraph, sometimes considered part of the chapter display, may include a note reference, though traditionalists will prefer an unnumbered note." ...and again in §2.20: "Avoid appending note references to chapter titles (see 1.48)." Here is what the APA Publication Manual 6th Edition says in §2.12, p.38: "Do not place footnote numbers in text headings." Here is what the Australian Style Manual 6th Edition says on p.209: "Avoid using note identifiers in headings. A note referring to an entire section, chapter or other component of a document should be unnumbered or unmarked and be placed before the numbered (or marked) notes at the base of the page, at the end of the chapter or article, or at the end of the book or document."
