Done from SB -> Styles -> Page panel, selecting a model style, and using the 'New from Selection' button action. However, was testing in current master against 7.1.0 and noticed that for preparing a template the page header/footer tabs will not honor clearing the for the Left, Right or First page: 'Same content on left and right' 'Same content of first' So an extra step of having to duplicate those coming from Standard template (and delete from template to avoid confusion). Once the different page styles are created--one for each header/footer needed--they are added to the template using the Insert break... dialog from menu Insert -> More Breaks -> Manual break. With each insert edit the header/footer the page style being defined. Use fixed text and fields as needed.
