Shouldn't we also handle EPOLLHUP here?  For example, isn't there a
deadlock if the user configures a notification eventfd, a notification
fires, the user closes the eventfd without reading it, and then tries
to swap in a different eventfd ctx?

Having this mutex held indefinitely waiting on the user to poll the
eventfd looks problematic.

Seems this is only needed because vfio_acpi_notify() takes an
acpi_handle, why didn't we use the solution from
vfio_remove_acpi_notify() and pass the whole adev?  vfio_acpi_notify()
could also return int and pass back an -ENOMEM since we've got the
