Don't worry, Trevor, I am still on it. See the links in comment #1 to track my progress.
My attempt to make sense of David's comment made me start to work on disentangling the layout and MIDI context hierarchies. As a result, in my opinion, the code crossed a complexity threshold that made it worthwile to also clean up other parts more extensively.
So I decided to work on my own branch "doc-ir-rewrite" for now, and to prepare a more complete patch set (or a merge offer) later. Since my time for this is limited, I have to ask for a bit of patience.
[Re-reading David's comment, I think I only now quite understand what he meant; the "engravers also used as performers" really threw me off. It is somewhat unfortunate that we could not sort this out earlier, and I may come back on this on Rietveld, but anyway I believe the direction I have taken makes sense.]
