Regarding the last comment: depending on how thin a wrapper around the PDF
imaging model Cairo is and what options it provides to provide part of the
PDF literally, my skepticism might not be warranted. Rendering through
something like Cairo would likely also give us a handle to bypassing
Ghostscript for rasterization altogether, generating bitmap files faster
and with fewer dependencies.
So it would likely be worth it if somebody (TM) evaluated Cairo for its
suitability here.
