Still leaving on review as I know this is a big patch and I would like
input from Hiekki and Trevor as it technically needs more work, but I'd
like to make sure apart from the making sure that the default MIDI and
\articulate.ly MIDI behaviour are clearly separated for readers, that there
is no technically inaccurate information there.
