I think we have thought about this enough and have an acceptable solution:
\fixed c'' { r8 a g a f a e a | d a a, a b, a cis a
| d a cis a d a e a | f1 }
where the octave marks on the reference pitch offset the octave of the
absolute-mode music inside. (And if for some reason someone puts one
\fixed inside another, the octaves of pitches are determined by the
innermost enclosing \fixed.)
