Issue 5468: yyout2grammar.py: Various fixes
The following problems are corrected with this commit.
* Bison Versions 2 and 3 produce slightly different output, which are now
both digested correctly.
* Continuation lines in both the `Terminals' and `Nonterminals' sections are
output in the correct place.
