No, LilyPond is not based on pixels at all. The default output formats (PDF, 
PS, EPS) are all vector graphic formats. The PNG/JPG output simply converts 
the resulting vector graphic to a pixel graphic. Unfortunately, this means 
that it is basically impossible to detect exactly where a note is printed in 
the pixel graphic.

I suppose it would be much easier to modify the .ly files and re-create the 
graphics...
