Did you follow the instructions on 
http://lilypond.org/stable/Documentation/windows/out-www/installing.html

In that step-by-step instruction, I found answers to a number 
of your questions. I agree, though, that it could be much clearer. 
Especially since the way of working is very different from the
point-and-click approach of most windows programs.
