What version of Lilypond are you using? If it occurs on 1.7.29, could you
provide a short example which demonstrates this?
If it's on 1.6.x, then please wait a few days or weeks until 1.8.0 is
released and then try it on that.

This is all possible to do in Lilypond with #'(extra-offset), although it
wouldn't be pretty. You might be able to define a new context that
does this all automatically, though. (sorry, my knowledge of Lilypond
doesn't extend that far)
