It's probably not very clear from the manual, but you can use the
\with{...} feature together with both syntaxes for creating a new
context:
\new Staff \with {SOME SETTINGS} {MUSIC}
and
\context Staff = NAME \with {SOME SETTINGS} {MUSIC}So, in your example, you probably want to do
PianoMD = {
\clef treble
\context Staff <<
\context Voice = one \with{...} \mdSuperiore
\context Voice = two \with{...} \mdInferiore

}The warning about the tremolo is harmless. It seems that the implementors didn't think of the case of a full bar tremolo, when they designed that warning. Since you get the correct output, it's nothing to worry about.
