With 2.3.20 #(set-default-paper-size "letter") seems to produce correct
dvi and ps but gives pdf's with the top cut off. Just setting papersize
= "letter" in the paper block (which probably does nothing) leaves
\def\lilypondpaperpapersize{a4}%
