This is a little simpler:

You could not use \general-align but the = would no longer line up with
the noteheads.
If you need to use the notation more than once, you could create a
variable:
crotchetequalscrotchet = \markup {
\fontsize #-1 {
\general-align #Y #DOWN \note #"4" #1
"="
\general-align #Y #DOWN \note #"4" #1
}
}
\version "2.18"
\relative c' {
\time 4/4
| c4 c c c
\tempo \crotchetequalscrotchet
\time 3/4
| c c c
\tempo \crotchetequalscrotchet
\time 4/4
| c1
}
