Hi Andrew,

The composer is Dirk Schäfer (1873 - 1931), in his time famous, now large 
forgotten. This is from No 5 of his "Interludes Op. 17". You can see and hear 
it on YouTube: https://www.youtube.com/watch?v=ycIdo0SK0GQ , starting at 9:13.
I think you will recognize that the notation, unusual as it may be, is very 
effective.
Reengraving it with LilyPond is not a walkover. In some places it has 8 voices.
