Extend your PianoStaff
\new PianoStaff <<
\new Staff = "upper" \upper
\new Staff = "lower" \lower
>>
to include a third staff, for example:
\new PianoStaff <<
\new Staff = "upper" \upper
\new Staff = "middle" \middle
\new Staff = "lower" \lower
>>
For that to work, you have to define middle's music too,
just like the music for upper and lower is already.
